UB04 Implementation and Information

The Kentucky State Uniform Billing Committee (KUBC) was formed to handle state implementation and to disseminate state UB manuals, which reflect the national guidelines and unique state billing requirements. 
 

Effective May 23, 2007, the UB-92 form/data set is discontinued and the final transition to the UB-04 will be complete. The final UB-04 data specifications manual is being finalized and will be available for purchase from the National Uniform Billing Committee (NUBC). Therefore as the industry will now move to a national Uniform Billing manual, the Kentucky UB-92 manual will be phased out.  The new format still has a state specific section to include instructions about unique state requirements for handling the reporting of workers’ compensation programs or other state level data that falls outside of the HIPAA transaction rules
 

The Kentucky Hospital Association (staff for the Kentucky Uniform Billing Committee) has entered into a partnership with the American Hospital Association, chair of the NUBC, to distribute the new UB-04 Data Specifications Manual. The manual is designed as a portable document that allows users easy maneuvering and referencing.
